Every DQ file needs six core documents for any driver: the employment application, a motor vehicle record, a road test certificate or license waiver, a medical examiner's certificate, previous employment verification, and an annual review of the driving record.
CDL drivers need three additional items on top of that: a copy of the current CDL, a pre-employment drug test result, and a Drug and Alcohol Clearinghouse query. Some drivers also need conditional documents, like entry-level driver training certificates or Skill Performance Evaluation certificates, depending on when they were licensed or whether they hold a medical waiver.
